Scheduling
Should I schedule a telehealth visit or an in-person visit?
If you're sick with COVID-19 symptoms, please schedule a telehealth visit or go to an urgent care clinic for testing. Otherwise, if the reason for your visit requires physical assessment, you need a blood pressure check, or injection teaching, please schedule an in-person visit.
How do I schedule?
Click the scheduling button. Once you have signed up for an account through Charm, you can schedule online. If you need assistance with scheduling, you can call 541-638-0261 and your call will be returned within 1 business day.
How do I reschedule or cancel an appointment?
Please call 541-638-0261 to cancel or reschedule your appointments with at least 24 hours notice.
Are there charges for late cancellations or missed appointments?
Yes. There is a $45 fee for missed appointments and for late cancellations occurring less than 24 hours before the scheduled appointment time.

How do I get medication refills?
Request refills directly from your pharmacy 5 business days prior to running out of your medication.
How do I get a form filled out and a letter written?
Schedule an appointment. For forms, email the completed and signed patient portion of the form prior to the visit. For letters, email the needed content prior to the visit.
How do I reach Kelly between appointments?
Email via the Charm Patient Portal. Please allow up to 5 business days for a reply. For emergencies, go to the nearest UrgentCare clinic or Emergency Department. For questions regarding new health concerns, please schedule an appointment- Kelly is unable to treat new medical conditions or prescribe new medications without an appointment.
